About Our Office


We at First Internal Medicine specialize in comprehensive diagnostic evaluation of complex medical problems and diligent management of chronic diseases. We focus on the root causes of ailments and subsequently offer highly personalized medical care. Our services are targeted for adults and teens (age 15 and older).

Our preventive services aim to help healthy individuals maintain their well-being and prevent disease through education about healthy lifestyle, proper nutrition and exercise. We focus on early detection (screening) and prevention of health problems such as atherosclerosis, heart disease, high blood pressure, high cholesterol, diabetes, obesity, osteoporosis and cancer. We consider sexual and mental health as an integral part of every preventive visit. We also offer pre-employment, school, sports and general physical exams, travel advice and full range of immunizations.

Our disease management services include evaluation and treatment of common ailments (colds, flu, sinus problems, bronchitis, diarrhea, back problems) as well as serious ones (such as heart disease including heart failure, asthma, COPD, diabetes, stroke, endocrine problems).

As internal medicine specialists we have particular expertise in diagnosis and management of complex medical conditions including those (e.g. chronic fatigue, chronic digestive problems, medically unexplained pain) that may perplex other primary care providers. We achieve this by formulating an extensive list of possible causes (differential diagnosis) followed by a step-wise evaluation of each possibility utilizing an extensive array of tests, imaging and consultations with appropriate specialists (locally but also nation-wide if necessary). Once a diagnosis and treatment plan is made, we follow our patients frequently to accelerate recovery and prevent complications. 

Regardless of the presenting problem, we strive to empower all our patients to actively participate in the healing process and prevention through extensive education about their condition, treatment options and prognosis. We hope that at each visit our patients gain health and nutritional wisdom that can be applied for years to come.

First Internal Medicine has changed to a membership based model of care (also called ‘concierge medicine’). In order for you to become a patient in the practice, you will need to subscribe to one of the two membership options outlined below. If you do not become a member, we will not be able to accept you in the practice.
 
The membership model is necessary in order for me to provide the following services which, in my opinion, are essential components of excellent medical care but are not reimbursed by traditional health insurance:

First of all, I need to restrict my patient panel so that I can be available to see you for any emergencies on a short notice, and to be able to spend enough time with you to address all your health concerns regardless of their number or complexity.
 
I want to continue providing excellent service by keeping you, the patient, as my first priority rather than surrendering to health insurance companies’ requests to limit cost in exchange for a payment from them, which insurance companies call ‘shared savings’.
 
I also want to make myself available to you via telephone, email and videoconferencing to save you time by addressing simple health issues without an office visit.
 
Finally, I want to offer more integrative, holistic approach to care by incorporating advanced diagnostic techniques (including genetic testing), intravenous hydration, vitamins, hormone replacement therapy and intravenous pain therapy to personalize your health care.
 
We are offering two membership levels for you to choose from, with different services included at a different price:
 
MEMBERSHIP CARE - $1,000 per person annually, or $275 quarterly

PERSONAL VIP CARE - $3,000 per person, $4,500 for 2 adults or $5,500 for a family
(2 adults and up to 2 dependents), billed annually.

The most important difference between the two options is that, in exchange for higher annual payment, Personal VIP Care members will have the ability to contact me via my personal cell phone 24 hours a day, 7 days a week, 365 days a year.
 
Second most important differentiation is that fee for Personal VIP Care includes all copays, deductible or coinsurance for most services available in the office.
 
The Basic Membership fee covers services that are not typically covered by insurance such as consultations via telephone or email (without an office visit), open slots in the schedule in order to accommodate same day appointments for Urgent Care, longer appointments to allow discussion of multiple problems at one visit, etc. In order utilize Basic Membership you must have an active health insurance or Medicare part B which will be billed for all covered services. WE DO NOT ACCEPT MEDICAID.
 
In order to provide exceptional care to the members, I limit the size of my patient panel to 400 patients, and lower the panel size further as the Members become VIP Care patients. We enroll new Members on the first-come first-served basis.

Thursday      Hospital day (phone and urgent care appointments only)
Friday           9 am - 1 pm (phone and urgent care appointments until 5 pm)
Saturday & Sunday - as needed for Personal VIP Care members only

Home visits (house call) before or after office hours as well as on weekends are available for Personal VIP Care members at nominal additional charge.
