I WOULD LIKE TO CLARIFY TWO POINTS THA HAVE CREATED A LOT OF QUESTIONS:
1) YOU CAN SIGN UP FOR THE MEMBERSHIP PROGRAM AFTER MARCH 20TH (SUBJECT TO AVAILABILITY).
2) THE LETTER THAT YOU HAVE RECEIVED IS NOT A TERMINATION LETTER. WE WILL CONTINUE TO PROVIDE CARE AFTER MARCH 20TH FOR PATIENTS WHO HAVE NOT SIGNED UP FOR MEMBERSHIP. OUR SERVICES WILL STOP ONLY AFTER YOU RECEIVE A SECOND, TERMINATION LETTER, WHICH WILL SPECIFY WHEN EXACTLY OUR CARE ENDS. We will make sure that you have adequate time to find a new provider.
This information is in the letter (second page, 4th paragraph from the top) but it may be overlooked.
February 18, 2016 – Our practice is at the crossroads. I have been deliberating for a long time whether to continue relying solely on health insurance (or Medicare) reimbursement for my services and let my practice turn into one of the many primary care practices where a provider would see four patients each hour and 25 patients per day, or continue following the principles of quality medicine (as outlined lower on this page) that have been guiding our practice for the last 8 years, even if that means charging patients extra for the time and effort. In the last year I have experimented with a concierge medicine model, offering it as an optional service. The response from patients has been extremely positive.
Therefore, we are changing the business of our practice to a membership-based (concierge medicine) model only. All existing patients who wish to remain in the practice, as well as all new patients, will need to enroll in one of the two membership programs and pay a yearly fee (retainer).

Members in each program will benefit from longer appointments (30-60 minutes), guaranteed urgent care appointments within 24 hours, free telephone consultations for minor medical problems, and email access to the physician.
Only Personal VIP Care members will have unlimited 24/7 access to Dr Huzicka’s cell phone with ability to Face Time or Skype.
Personal VIP Care membership fee includes all services available in the office (EKG, spirometry, urine testing, Strep/influenza/pregnancy tests) as well as basic laboratory tests (blood count, comprehensive metabolic panel, urinalysis, thyroid, HIV) without additional charges. Therefore, Personal VIP Care program is a cost-effective option for individuals with high-deductible (catastrophic) health insurance plans, and for those without health insurance.
Additional benefits of Personal VIP Care include expanded preventive screenings, expedited referrals and prior authorizations, standard and basic travel vaccines, and home visits (at nominal additional charge).
Move to concierge medicine will allow us to focus even more on each individual patient’s health, and eventually offer cutting-edge technology such as genomic testing and stem-cell therapy.
All existing patients will need to enroll in the membership program in order to remain in the practice. We plan to limit enrollment to 400 members in the basic program and approximately 100 members in the Personal VIP Care program in order to ensure quality service for all members. New patients will be accepted to the practice only until the quotas are met.
We at First Internal Medicine specialize in comprehensive diagnostic evaluation of complex medical problems and diligent management of chronic diseases. We focus on the root causes of ailments and subsequently offer highly personalized medical care. Our services are targeted for adults and teens (age 15 and older).
Our preventive services aim to help healthy individuals maintain their well-being and prevent disease through education about healthy lifestyle, proper nutrition and exercise. We focus on early detection (screening) and prevention of health problems such as atherosclerosis, heart disease, high blood pressure, high cholesterol, diabetes, obesity, osteoporosis and cancer. We consider sexual and mental health as an integral part of every preventive visit. We also offer pre-employment, school, sports and general physical exams, travel advice and full range of immunizations.
Our disease management services include evaluation and treatment of common ailments (colds, flu, sinus problems, bronchitis, diarrhea, back problems) as well as potentially serious ones (chest pain, shortness of breath, abdominal pain, stroke symptoms). However, as internal medicine specialists we have particular expertise in diagnosis and management of complex medical conditions. We achieve this by formulating an extensive list of possible causes (differential diagnosis) followed by a step-wise evaluation of each possibility utilizing an extensive array of tests, imaging and consultations with appropriate specialists. Once a diagnosis and treatment plan is made, we follow our patients frequently to accelerate healing and prevent complications.
Regardless of the presenting problem, we strive to empower all our patients to actively participate in the healing process and prevention through extensive education about their condition, treatment options and prognosis. We hope that at each visit our patients gain health and nutritional wisdom that can be applied for years to come.
